Certain dietary patterns are associated with GLIM criteria among Chinese community-dwelling older adults: a cross-sectional analysis

Disease-related malnutrition is prevalent among older adults; therefore, identifying the modifiable risk factors in the diet is essential for the prevention and management of disease-related malnutrition. The present study examined the cross-sectional association between dietary patterns and malnutrition in Chinese community-dwelling older adults aged ≥65 years in Hong Kong. Dietary patterns, including Diet Quality Index International (DQI-I), Dietary Approaches to Stop Hypertension (DASH), the Mediterranean Diet Score, ‘vegetable–fruit’ pattern, ‘snack–drink–milk product’ pattern and ‘meat–fish’ pattern, were estimated and generated from a validated food frequency questionnaire. Malnutrition was classified according to the modified Global Leadership Initiative on Malnutrition (GLIM) criteria based on two phenotypic components (low body mass index and reduced muscle mass) and one aetiologic component (inflammation/disease burden). The association between the tertile or level of adherence of each dietary pattern and modified GLIM criteria was analysed using adjusted binary logistic regression models. Data of 3694 participants were available (49 % men). Malnutrition was present in 397 participants (10⋅7 %). In men, a higher DQI-I score, a higher ‘vegetable–fruit’ pattern score and a lower ‘meat–fish’ pattern score were associated with a lower risk of malnutrition. In women, higher adherence to the DASH diet was associated with a lower risk of malnutrition. After the Bonferroni correction, the association remained statistically significant only in men for the DQI-I score. To conclude, a higher DQI-I score was associated with a lower risk of malnutrition in Chinese older men. Nutritional strategies for the prevention and management of malnutrition could potentially be targeted on dietary quality.
Urinary sodium excretion and dietary sources of sodium intake in community-dwelling Chinese adults aged 50 years and over

Objective:
This study examined Na intake and identified the major food sources of Na in healthy Chinese adults aged ≥50 years in Hong Kong.
Design:Participants who completed a baseline assessment of a randomised controlled trial assessing the effectiveness of exercise and nutrition supplementation on improving physical and cognitive functions were included. A single 24-h urine sample and a 3-d diet record were used to measure urinary Na excretion and dietary intake, respectively. The compliances to the WHO (<5 g/d) and China Nutrition Society (<6 g/d) recommendations for salt intake were assessed based on the urinary Na excretion. The relative contribution of sixteen food groups to the dietary Na intake was expressed as percentages. Associations between food groups and urinary Na excretion were analysed using multiple linear regression.
Setting:Community facilities.
Participants:Totally, 114 healthy Chinese adults (mean age 60·6 years, 55 % women).
Results:The mean urinary Na excretion over 24-h was 2876·6 ± 1249·4 mg/d (7·3 ± 3·2 g salt/d). Overall, 22·8 % of participants met the WHO recommendation and 34·2 % met the Chinese Nutrition Society recommendation for salt intake. The major food sources of dietary Na intake were condiments (42·4 %), cereals and their products (16·8 %) and soups (13·5 %). Higher intakes of seafood and nuts were associated with lower urinary Na excretion.
Conclusions:Public health strategies should target healthy Chinese adults in Hong Kong to modify the current patterns of Na intake. Targeting condiments, cereals and their products and soups will be an important strategy to reduce their Na intake.

The productivity of riparian sites in managed forests can be focused to provide productive fish and wildlife habitat while yielding most of its productive capacity for other than amenity values. Establishment of habitat protection goals and measures of achievement permit flexible approaches for meeting them. Once the protection standards are set, intensive management of the woody cover is logically dependent on minimum disturbance methods, in general, for both vegetation management and harvest. Several currently registered chemical products and non-chemical methods are helpful and safe in achieving both yield and protection goals.

This cross-sectional study examined dietary patterns, and the associations of these patterns with demographics, lifestyle, anthropometry and blood pressure in 3707 Chinese people aged 65 years and above taking part in a population-based cohort study investigating the risk factors for osteoporosis. Baseline dietary data were collected using a validated FFQ. Dietary patterns were identified using factor analysis. Scores were calculated for each pattern. Demographics, lifestyle factors and self-reported hypertension history were collected through a questionnaire. BMI, waist circumference (WC), hip circumference (HC), waist-to-hip ratio, systolic blood pressure and diastolic blood pressure (DBP) were measured. Three dietary patterns were identified, namely ‘vegetables–fruit’, ‘snacks–drinks–milk products’ and ‘meat–fish’. Participants who were more physically active, more educated, non-smokers and non-drinkers were more likely to have higher ‘vegetables–fruit’ dietary pattern scores. Current smoking habit and alcohol use were associated with higher ‘snacks–drinks–milk products’ dietary pattern scores and ‘meat–fish’ dietary pattern scores. ‘Vegetables–fruit’ dietary pattern scores were inversely (unstandardised regression coefficient B = −0·60 mmHg, 95 % CI −1·04, −0·16) and ‘snacks–drinks–milk products’ dietary pattern scores were positively (B = 0·50 mmHg, 95 % CI 0·08, 0·92) associated with DBP in men in multiple regressions. Higher ‘meat–fish’ dietary pattern scores were associated with higher BMI (B = 0·19 kg/m2, 95 % CI 0·06, 0·33), waist-to-hip ratio (B = 0·004, 95 % CI 0·002, 0·007) and WC (B = 0·57 cm, 95 % CI 0·18, 0·97) in men, and higher BMI (B = 0·40 kg/m2, 95 % CI 0·22, 0·57), WC (B = 0·87 cm, 95 % CI 0·39, 1·36) and HC (B = 0·61 cm, 95 % CI 0·26, 0·96) in women in multiple regressions. The influence of demographic and lifestyle characteristics on dietary patterns and the health risks associated with dietary patterns provides insights for the provision of tangible dietary advice to this population.

Objective
To study the relationship between dietary intake and the development of type 2 diabetes among Chinese adults.
DesignA prospective cohort study. Dietary assessment was carried out using a validated FFQ. Principal component analysis was used to identify dietary patterns. Dietary glycaemic load and variety of snacks were also calculated.
SettingA hospital-based centre at the Queen Mary Hospital in Hong Kong SAR, China.
SubjectsA total of 1010 Chinese adults aged 25–74 years who participated in a territory-wide dietary and cardiovascular risk factor prevalence survey in 1995–1996 were followed up for 9–14 years for the development of diabetes.
ResultsA total of 690 (68·3 %) individuals completed follow-up during 2005–2008 and seventy-four cases of diabetes were identified over the follow-up period. Four dietary patterns were identified (‘more snacks and drinks’, ‘more vegetables, fruits and fish’, ‘more meat and milk products’ and ‘more refined grains’). After adjustment for age, sex, BMI, waist-to-hip ratio, smoking, alcohol intake, participation in exercise/sports and family history of diabetes, the more vegetables, fruits and fish pattern was associated with a 14 % lower risk (OR per 1 sd increase in score = 0·76; 95 % CI 0·58, 0·99), whereas the more meat and milk products pattern was associated with a 39 % greater risk of diabetes (OR per 1 sd increase in score = 1·39; 95 % CI 1·04, 1·84). Dietary glycaemic load, rice intake, snack intake and variety of snacks were not independently associated with diabetes.
ConclusionsThe more vegetables, fruits and fish pattern was associated with reduced risk and the more meat and milk products pattern was associated with an increased risk of diabetes.

The objective of the present paper is to provide glycaemic index (GI) and glycaemic load (GL) values for a variety of foods that are commonly consumed in Hong Kong and expand on the international GI table of Chinese foods. Fasted healthy subjects were given 50 g of available carbohydrate servings of a glucose reference, which was tested twice, and test foods of various brands of noodles (n 5), instant cereals (n 3) and breads (n 2), which were tested once, on separate occasions. For each test food, tests were repeated in ten healthy subjects. Capillary blood glucose was measured via finger-prick samples in fasting subjects ( − 5, 0 min) and at 15, 30, 45, 60, 90 and 120 min after the consumption of each test food. The GI of each test food was calculated geometrically by expressing the incremental area under the blood glucose response curve (IAUC) of each test food as a percentage of each subject's average IAUC for the reference food. GL was calculated as the product of the test food's GI and the amount of available carbohydrate in a reference serving size. The majority of GI values of foods tested were medium (a GI value of 56–69) to high (a GI value of 70 or more) and compared well with previously published values. More importantly, our dataset provides GI values of ten foods previously untested and presents values for foods commonly consumed in Hong Kong.

Environmental and lifestyle factors that affect oxidative stress and inflammation may influence telomere length (TL). There are limited data to relate the effect of dietary components on TL. The present study examined the association between food groups and TL in a sample of elderly Chinese. In a sample of 2006 Chinese (976 men and 1030 women) aged 65 years and over, TL was measured by quantitative real-time PCR and daily intake of food groups was assessed by a validated FFQ. Linear regression and analysis of covariance were used to examine the association between food group intake and TL, with adjustment for demographic and lifestyle factors. In men, only Chinese tea consumption was significantly associated with TL after adjustment for demographics and lifestyle factors (P = 0·002). Mean difference in TL for those in the highest quartile of Chinese tea consumption (>3 cups/d or >750 ml/d) as compared with those in the lowest quartile of Chinese tea consumption ( ≤ 0·28 cups/d or ≤ 70 ml/d) was 0·46 kb, corresponding to approximately a difference of 5 years of life. In women, intake of fats and oils was borderline and negatively associated with TL after adjustment for demographic and lifestyle factors (P = 0·037). In conclusion, Chinese tea consumption was positively associated with TL in elderly Chinese men.
Bone mineral status and its relation with dietary estimates of net endogenous acid production in Hong Kong Chinese adolescents

Diet composition influences net endogenous acid production (NEAP), which may affect bone health. No studies are available to relate dietary estimate of NEAP to bone health in Chinese adolescents. This study examined the association of dietary estimates of NEAP with bone mineral status in Hong Kong Chinese adolescents. Baseline data on 171 boys and 180 girls aged 10–12 years from the Hong Kong Adolescent Bone Health Cohort Study were presented. Weight, height, Tanner stage and dietary intakes by FFQ were collected. NEAP was estimated from diet using Frassetto's method. Bone area (BA), bone mineral content (BMC) and bone mineral apparent density (BMAD) of total hip, lumbar (L1–L4) spine and whole body were estimated by dual-energy X-ray absorptiometry (DXA). No significant association was observed between BMC or BMAD and energy-adjusted NEAP or other nutrients. BA was significantly and positively associated with BMC at all sites in both sexes. Weight was significantly and positively associated with BMC in hip and spine in both sexes. Height was negatively correlated with hip BMC for boys and whole body BMC for girls. Pubertal stage was significantly and positively associated with BMC in all sites in both sexes. Weight and height contributed most of the variability in BMAD at different sites. The results suggest that anthropometric characteristics and pubertal stage are more influential than dietary NEAP in determining bone mineral status of Hong Kong Chinese adolescents. However, the methodological weaknesses regarding the use of DXA and FFQ in the present sample require attention.
